Customer Forms

We have provided several forms below for our customers' needs.  Most of these forms have been saved as a PDF file.  You will need to download or have a copy of Adobe Acrobat Reader 4.0 or better on your computer in order to view it.  In order to download the latest version of Adobe Acrobat Reader, please access Adobe's website.

Water Service Agreement Card	Are you new to the area and would like to sign up for service, but cannot make it into our office. We now have our Water Service Agreement Card available for printing. The agreement card is required for all new water accounts - simply print down, fill out, and mail to our office at 205 Billings Street.
Residential Waiver of Deposit & Authorizaton of Direct Deposit of Billing	New to the area?  Never had water service with us?  We will waive the required water security deposit if you sign up for ACH / Direct Deposit and allow us to deduct your water bill from your checking account.  Simply print down this form, along with the ACH / Direct Debit form, fill out and bring to our office and we will get you signed up.
ACH / Direct Debit - Water Bill Payment	So you wish to avoid the hassle of mailing in your water bill payment?  Don't have time to come to our office to make your payment? Do you wish you could have your water bill deducted from your checking account?  Then print down BOTH PAGES of this handy form, fill out and either mail or drop off at our office and we will do the rest!
ACH / Direct Debit Cancellation	Do you wish to cancel your ACH / Direct Debit account?  Are you changing banks and wish to continue ACH / Direct Debit on your new account?  Then please fill out this form and either mail or drop off at our office.
Social Security - Late Fee Waiver	Are you on Social Security?  Do you receive your Social Security payment AFTER the water bill is due?  Are you always seeing a late penalty on your water bill?  Then simply print down and fill out this form, and either mail or drop off at our office, and we will waive the late penalty on all your bills.
Deposit Transfer	Do you wish to transfer your water security deposit and water bill to someone else due to remarriage, divorce, etc.?  Do you wish to change the name on your water bill?  Then simply print down and fill out this form, and either mail or drop off at our office.  We will notarize the form (free of charge) and complete the transfer.
Deposit Transfer-Customer Deceased Form	Do you wish to transfer the water security deposit and water bill into your name due to a death in the family?  Then simply print down and fill out this form, and either mail or drop off at our office.  We will notarize the form (free of charge) and complete the transfer.
